Bahaya Duduk Kelewat Lama

Di bandara internasional umumnya disiapkan klinik siaga untuk menangani kasus sumbatan pembuluh darah balik paru-paru, deep vein thrombosis. Pasien mendadak terserang sesak napas akibat ada butiran atau bekuan darah yang menyumbat pipa pembuluh darah balik (vena) dari tungkai dan kaki yang menuju ke paru-paru. Sumbatan dadakan itulah yang menimbulkan serangan sesak napas. Penderitanya bisa meninggal mendadak bila tak segera ditolong.

Betah duduk
Ya, betul. Jangan duduk kelewat lama. Terlebih dengan posisi duduk bersempit-sempitan. Mengingat kasus seperti ini lebih sering-terjadi di pesawat lintas benua kelas ekonomi (karena kursinya sempit dan susah selonjor), penumpangnya perlu lebih sering rehat berdiri setiap beberapa jam.

Di kabin pesawat juga ada panduan untuk itu. Setiap satu dua jam biasakan untuk bangkit berdiri dari posisi duduk. Lakukan pergerakan tungkai dan kaki sambil berjalan mondar-mandir di ruang kabin pesawat. Maksudnya agar tidak sampai terjadi aliran darah yang tidak lancar, selama dalam posisi duduk tanpa bergerak.

Dalam posisi duduk di kursi sempit, sehingga tungkai dan kaki sukar berubah posisi atau digerak gerakkan, kondisi pipa pembuluh darah balik terancam, aliran darahnya mengalir dengan kederasan yang sama lemahnya. Jika kondisi darahnya dan atau dinding pipa pembuluh darah baliknya berisiko untuk terjadinya DVT, posisi duduk lama itu yang menjadi pemicunya.

Hal yang sama juga dapat terjadi selama bekerja di kantor atau di rumah yang mengharuskan duduk berlama-lama. Perlu bangkit dari posisi duduk,dan selama duduk posisi tungkai dan kaki diubah-ubah atau digerak-gerakkan, agar tidak terpicu kejadian DVT (deep vein thrombosis).

Deep vein thrombosis terjadi akibat terbentuknya bekuan darah, hasil kumpulan sel-sel yang berisiko untuk luruh, lalu hanyut terbawa aliran darah. Biasanya berasal dai pembuluh darah tungkai dan kaki.

Namun, bukan pembuluh darah balik di permukaan kulit yang berisiko mengalami hal seperti itu, melainkan pembuluh darah balik (vena) di bagian dalam.

Karena sistem pembuluh darah balik itu mengalirkan darah dari bagian tepi tubuh menuju ke paru-paru (darah kotor), jika terjadi luruh dan terhanyutnya butiran atau bekuan darah, bahan penyumbat akan tersangkut di pipa pembuluh darah paru. Sumbatan di sini yang menimbulkan serangan sesak napas mendadak.

Mekanisme pembentukan butiran atau bekuan darah tentu bukan kejadian satu-dua hari. Prosesnya sudah berlangsung lama, sebelum serangan DVT muncul. Bisa jadi memang sudah ada "bibit" ke arah situ.

Tak selalu mudah mengetahui seseorang sudah memiliki bibit bakal terhanyutnya bekuan atau butiran darah dari tungkai dan kaki. Tanpa pemeriksaan seksama, belum tentu dikenali. Sepintas tak dapat dikenali kalau sudah ada gangguan itu di pipa pembuluh darah balik, bila tidak dilakukan pemeriksaan.

Ada bendungan pembuluh balik
Jika betul ada pembentukan karat di pipa pembuluh darah balik (thrombotic), berarti aliran darah tidak selancar pada pipa pembuluh darah balik yang masih normal. Tergantung seberapa berat atau tidak lancarnya aliran darah balik menuju ke paru-paru, keluhan mungkin belum begitu berarti.

Pada mereka yang sudah mengalami kondisi sumbatan di lokal pipa pembuluh (post thromboticsyndrome), mungkin hanya muncul keluhan pegal tungkai menahun, kadang pembengkakan tungkai, lekas letih pada tungkai, selain kemungkinan terbentuknya borok di tungkai dan atau kaki yang sukar sembuh (skin ulcer).

Namun, keluhan seperti tersebut di atas bukan cuma milik kasus pembuluh balik bermasalah saja. Banyak kelainan dan penyakit memunculkan keluhan serupa. Kekurangan vitamin B, gangguan ginjal atau hati, varises, sukar dibedakan dengan sindroma thrombotic ini.

Karena kemungkinan bekuan dan "karat" yang melekat pada dinding pembuluh darah balik sewaktu-waktu bisa luruh dan terlepas (embolism), sehingga menjadi venous thromboembolism (VTE), diperlukan obat antibeku darah (antikoagulan). Sekiranya sampai terjadi bekuan atau butiran darah terhanyut, akan segera dilarutkan oleh obat pelarutnya itu, sehingga serangan sesak napas dapat dibatalkan.

Pemeriksaan USG dengan Doppler
Sekarang dapat lebih dini dikenali adanya gangguan dalam pipa pembuluh darah balik tungkai dan kaki. Caranya, memanfaatkan USG khusus dengan alat doppler. Sudah terbentuknya thrombotic dapat dideteksi dengan cara ini. Itu berarti bahwa kecolongan terjadi-nya kasus DVT yang berakhir dengan kematian mestinya dapat diantisipasi, dan dilakukan upaya pencegahan jauh-jauh hari sebelumnya.

Yang berisiko terserang DVT tentu saja mereka yang memiliki kelainan darah ("darah kental"), yang kadar lemak darahnya tinggi, mengidap penyakit metabolisme, sehingga kadar lemak darah memburuk. Penderita tensi darah tinggi juga perlu lebih mewaspadainya.

Keluhan yang sering muncul di tungkai tanpa penyebab jelas perlu diwaspadai kalau-kalau memang sedang atau sudah ada gangguan di dalam pipa pembuluh darah balik tungkai dan kaki. Mereka ini tergolong berisiko dan memerlukan pemeriksaan lanjutan untuk memastikan kalau benar tengah mengidapnya.

Teknik melarutkan bekuan
Bila sudah telanjur terjadi serangan trombosis, dapat dilakukan tindakan untuk mengangkat bekuan trombusnya itu. Pada kasus serangan trombosis dadakan (acute venous trommbasis) dapat dilakukan tindakan menggunakan selang kateter (ATTRACT = thrombosis removal with adjunctive catheter directed thrombolysis).

Selang dimasukkan langsung ke dalam pembuluh balik, dan obat pelarutnya diberikan sekaligus ke dalam pipanya. Melihat makin banyak saja kasus DVT sekarang ini, khususnya di bandara, hendaknya saat turun dari pesawat terbang, kehati-hatian perlu lebih ditingkatkan.

Di Amerika Serikat tercatat tak kurang ada 100 ribu kasus DVT setiap tahunnya (Dr Steven Galson). Mengingat upaya pencegahan dapat dilakukan, pihak medis tak perlu sampai kecolongan hingga menelan korban jiwa.

Ungkapan "kalau sudah duduk lupa berdiri" sebagai sindiran politik buat kalangan eksekutif, ternyata juga punya makna medis. Menduduki posisi, maupun duduk dalam arti sesungguhnya, betul tidak boleh terlampau lama, apalagi berlama-lama.
